Please be advised that Los Angeles Department of Water & Power (LADWP) intends to use a consumer and/or
investigative consumer report as part of your continued access to Bulk Electric System (BES) Critical Cyber
Assets during the course of your employment with LADWP. Your consent for the procurement and use of such a
report is required for continued access to BES Critical Cyber Assets. The consumer report will contain
information from public records, which may include, but are not limited to, social security number and
address history, criminal history and resources disclosing sanctions, debarments and exclusions from
government and other agency databases, to the extent permitted by law. You must provide authorization for
this background check to be conducted. This will be done via an electronic signature.

Any consumer report or investigative consumer report prepared will be used strictly for employment purposes
as defined in the Fair Credit Reporting Act, as a report to be used for the purpose of continued access to
BES Critical Cyber Assets. Any continued access to BES Critical Cyber Assets will be conditional upon the
receipt of satisfactory informationAn electronic signature shall be valid as the original.
This investigation will be conducted in compliance with federal law, which provides specific protection to
you, as stated in the following document: Summary of Your Rights Under the Fair Credit Reporting Act
prepared pursuant to 15 U.S.C.Section 1681-1681u.
