Background Screening Authorization and Instructions
In connection with my request for an independent contractor assignment at CITY OF PHOENIX WATER SERVICES DEPARTMENT, I understand that a consumer report, as that term is defined in the Federal Fair Credit Reporting Act as amended ("FCRA"), 15 U.S.C. 1681 et seq. and applicable state law, may be obtained by CITY OF PHOENIX WATER SERVICES DEPARTMENT from a consumer reporting agency. You must provide authorization for this background check to be conducted. This will be done via an electronic signature.
I hereby authorize CITY OF PHOENIX WATER SERVICES DEPARTMENT now, or at any time while I am an independent contractor at CITY OF PHOENIX WATER SERVICES DEPARTMENT, to obtain a consumer report on me. The consumer report will contain information from public records, which may include, but are not limited to, social security number, motor vehicle operation history, and criminal history, to the extent permitted by law.
I further understand that, upon my written request, I will be given the name and address of each consumer reporting agency from which a consumer report or investigative consumer report may have been obtained, and, if an investigative consumer report has been obtained, a description of the nature and scope of the investigation. I understand that I may obtain additional information concerning the report by contacting the consumer reporting agency
